The Integral That Changed Medicine

In the 1960s, pharmacologists faced a puzzle: two formulations of the same drug at the same dose produced wildly different effects. The breakthrough came when they started measuring area under the curve (AUC) — the integral of blood concentration over time.

A fast-release pill spikes high then crashes. A slow-release version stays moderate for hours. The AUC can be identical, but the shape matters for safety. Integrate the spike: too much drug at once can be toxic. Integrate the sustained curve: steady therapeutic levels with fewer side effects.

Today FDA approval of a generic drug hinges on AUC bioequivalence — proving its concentration curve matches the original closely enough to be interchangeable.
